I have noticed several players have played over a thousand games yet are still low level. Does this indicate that the ladder has been reset recently? If so, what is the schedule, and if not, how do these people remain low level.

The current leveling system was only implemented at the end of 2009. While many have been playing matches for years already, rising to the top of the experience list has only been possible through games of the past month and a half.

It's because level system was introduced only a while ago. The wins/loss/draw system is the old system which was present from the beginning.
